Quartz is seeking a compassionate, skilled Health Coach to join our Clinical Service department and support our mission of empowering members to live healthier, more fulfilling lives. In this role, you will provide personalized health coaching, facilitate behavior change, and guide members as they navigate their wellness journey. Responsibilities Facilitate telephonic or virtual one‑on‑one coaching sessions using motivational interviewing and evidence‑based behavior change strategies. Engage high‑risk or vulnerable members through agile outreach to promote preventive services and build trusting, long‑term relationships. Coordinate referrals to medical, behavioral health, and social/community‑based resources. Conduct initial consultations and whole‑person assessments, evaluating health history, lifestyle habits, social determinants of health, readiness to change, and personal goals. Collaborate with members to develop individualized wellness plans that reflect their unique needs, strengths, and preferences. Provide clear, evidence‑based education on nutrition, physical activity, stress management, sleep hygiene, preventive care, and chronic disease management. Support informed decision‑making by sharing credible health information and connecting members with relevant tools and resources. Track progress and document coaching sessions within electronic health records (EHR), CRM systems, or digital coaching platforms. Qualifications Bachelor’s or Associate’s degree in health education, nutrition, exercise science, psychology, kinesiology, public health, or a related field. 2+ year of experience providing one‑on‑one or group health coaching, wellness counseling, or comparable client support. Experience as a health/wellness coach, nurse, registered dietitian, or dietetic technician. Experience working in a health plan, managed care, or population health environment. Familiarity with health tracking apps, telehealth platforms, or virtual coaching systems. Understanding of behavior change frameworks (e.g., Transtheoretical Model, CBT principles, Health Belief Model). Bilingual proficiency strongly preferred. Excellent interpersonal, communication, and empathetic listening skills. Strong organizational, time management, and documentation skills. Proficiency with telehealth platforms and digital coaching tools. Motivational leadership: Encourages members to take ownership of their health.
