About the ECM Program

Our Enhanced Care Management (ECM) program delivers a whole-person, interdisciplinary approach to care, addressing the clinical and non-clinical needs of clients with complex medical and social conditions. The program focuses on:

  • Outreach and Engagement

  • Comprehensive Assessment & Care Planning

  • Enhanced Coordination of Care

  • Health Promotion

  • Transitional Care Services

  • Member and Family Support

  • Community Resource and Social Service Referrals

The ultimate goal is to improve care coordination, address social determinants of health (SDOH), and reduce unnecessary healthcare utilization for high-need Medi-Cal beneficiaries.

Job Overview

Reporting to the Program Manager, the Case Manager delivers high-quality, client-centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact.

Key Responsibilities

  • Conduct initial and quarterly psychosocial assessments

  • Collaborate with participants on individualized service plans and housing stability goals

  • Build trusted, professional relationships with clients

  • Provide outreach, housing navigation, and ongoing support

  • Facilitate referrals to healthcare, mental health, substance use, and community services

  • Support clients in obtaining employment, benefits, and education opportunities

  • Accompany clients to key appointments as needed

  • Use a harm reduction, strength-based, and client-focused approach
